Nurse Practitioners – Care Co-ordination, SPA, Oncology Helpline, Triage

 

Permanent role: £22.86 per hour plus enhancements

Based: Longbow Close, Shrewsbury

Hours: Part-Time, Monday to Sunday

We are currently recruiting additional people to join our very friendly team of healthcare professionals in our Care Co-ordination Centre at Longbow Close, Shrewsbury.

The role of the Nurse Practitioner will be to work in three blended specialities, Care Co-ordination (CCC) Single point of access (SPA) the Oncology helpline and Telephone triage.

CCC

The Care Co-ordination Centre (CCC) provides a unique telephone based service to primary, urgent and emergency care colleagues, across Shropshire and Powys, helping to co-ordinate, advise and re-direct via relevant pathways, arranging admissions to emergency departments, hospital specialties and community services. Working closely with our partners in community care services to respond proactively, assisting with admission avoidance and therapeutic interventions. We are particularly interested in hearing from Nurses with a community background who understand the pathways available for admission and admission avoidance.

Oncology

The Oncology helpline is a dedicated, professional helpline with access to a team of Nurses who are experts in this area. Following National Guidelines, endorsed by MacMillan Cancer Support and the UK Oncology Nursing Society (UKONS) the team provide a 24-hour helpline for patients under the care of Oncology and Haematology services from several locations across England and Wales.

The role can be challenging, but is also very rewarding. You will require a holistic approach, with the ability to show both compassion and empathy, with a calm and genuine telephone manner Patients and their relatives or carers can call for advice on any aspect of their condition or treatment, outcomes range from advice and reassurance to emergency hospital admission. We are particularly interested in hearing from Nurses with a background in oncology or haematology, who understand the specific issues affecting patients undergoing treatment in those specialties and the triggers for urgent hospital admission.

Telephone triage

Telephone triage/CCC SPA is an in and out-of-hours service, triaging calls received directly from 111 and other OOH providers. Telephone triage here at Shropdoc is not pathway lead and relies on the skill of the UCP to autonomously assess multiple presentations and undifferentiated conditions forming a clinical diagnosis and treatment strategy, through the use of critical thinking and evidence based practice to support this.  Early recognition of red flags is vital. Excellent verbal communication and interpersonal skills are essential, as is multidisciplinary working, with a practical knowledge of a wide range of clinical conditions, with a clear recognition of your limitations.
